In this role you will:

  • Function as a nutrition consultant, completing nutrition referrals from SHS providers.

  • Complete nutrition assessments and develop and implement nutrition care plans based on industry standards.

  • Perform nutrition counseling for patients/caregivers regarding formulas and /or therapeutic diets. This requires knowledge of composition of infant and pediatric formulas, along with the ability to calculate specialized formula recipes and meal plans.

  • Communicate nutrition assessments, counseling reports and care plans to referring physicians and other health care providers through the electronic medical record system (EHS).

  • Continually update knowledge recommended for nutritional therapies pertaining to specific diseases and disorders.

  • Coordinate nutritional care of patients with physicians and other health care providers (nurses, case managers, behavioral health, and hospital/community dietitians.

  • Participate in team huddles.

  • Serve as nutrition resource to medical team, health center staff and community.

  • Develops and nurtures relationships with outside services (YMCA, Food Banks, etc.) that will assist in providing education and resources to the patients and communities surrounding the health center sites.

  • Fulfill continuing education requirements to maintain registration (RD) through Commission on Dietetic Registrations (CDR), Pennsylvania State Dietitian/Nutritionist Licensure (LDN) through the Pennsylvania Division of Professional Regulation, and additional specialty certification (e.g., CNSC, CDE, CSP).

  • Comply with HIPPA and OSHA/safety regulations.

  • Support the concepts and philosophies of patient and family-centered care.

  • Adhere to the Spectrum Health Services standard of Behavior.

 

Experience and Skills:

  • Bachelor’s degree from an accredited university or college and course work accredited or approved by the Accreditation Council for Education in Nutrition and Dietetics (ACEND) of the Academy of Nutrition and Dietetics (AND) required; Master’s degree in Nutrition preferred.

  • Minimum of two (2) years experience in a primary care setting preferred.

  • RD certificate required, obtained by successfully completion of the national Registered Dietitian/Nutritionist examination administered by the Commission on Dietetic Registration (CDR).

  • Must obtain Dietitian/Nutritionist licensure (LDN) through the Delaware Division of Professional Regulation within 3 months of hire.

  • CDE, CNSC and/or CSP certification preferred.
