Job Description:

Spectrum Health Services is in search of a dynamic Behavioral Health Consultant to join its team of exceptional integrated primary care providers. Candidates must be one of the following: Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage & Family Therapist (LMFT), or Licensed Psychologist (PhD or PsyD) in the state of Pennsylvania with no restrictions to practice within the scope of their license, and ideally possess at least 3-5 years post Master's experience.

In this role, you will provide behavioral health consultations as part of a team-based model of integrated care delivery to primary care patients. You will partner with medical providers (Nurse Practitioners, Physicians, Dietitian, OB/GYNs, Dentists/Dental Hygienists) to improve the health, functioning, and self-management of chronic health conditions (both physical and mental health) in children, adolescents, adults, and older adults.

Essential Functions:

  • Provide professional expertise and consultation to Primary Care Providers and the care team within a population-based care framework.
  • Work collaboratively across disciplines within the organization and the surrounding community to identify and help manage 'at risk' 'chronically ill,' and 'frequent attenders.'
  • Perform strengths-based assessment and brief behavioral treatment for a wide range of psychological and substance abuse disorders. Refer to specialty clinics as appropriate.
  • Develop proficiency in fundamental medical terminology and education as it relates to chronic illness e.g. diabetes, hypertension, COPD/asthma, pain disorders
  • Work in a 'readiness to change' (Stages of Change) model versus a long term therapeutic model of care.
  • Apply knowledge of and experience with a broad repertoire of psychotherapeutic skills from a variety of theoretical perspectives (e.g. Motivational Interviewing, Solutions Focused Tx,, Mindfulness techniques, Acceptance & Commitment Tx, and CBT).
  • Facilitate support groups e.g. chronic disease-self management, pain management, smoking cessation.
  • Help patients, families, and caregivers build skills to improve coping, distress tolerance, and stress reduction.
  • Provide psychoeducation about preventative care and wellness as well as techniques to self-manage symptoms and chronic illness.
  • Develop collaborative relationships with external medical, behavioral, and social service community partners.
  • Provide diagnostic assessments and effective triage of patients in need of referrals to specialty mental health care
  • Develop educational, training materials and conducts sessions or disseminates information to care team on recent advances in the field.
  • Participate in Quality Improvement/Quality Assurance activities as appropriate.
  • Represent the heath agency at external events and meetings.

Experience and Skills:

  • Licensed Clinical Social Worker (LCSW) or Licensed Psychologist: PsyD or PhD,  Licensed Professional Counselor (LPC) or Licensed Marriage and Family Therapist (LMFT)
  • Experience in substance abuse counseling.
  • Familiarity with Medical Assisted Treatment (MAT).
  • Desire to work with diverse under-served populations. 
  • Comfort and flexibility in working in a fast-paced health environment.
  • Experience with interdisciplinary team-based care preferred.
  • Medical literacy and knowledge of psychopharmacology desired.

· $1,000 tuition reimbursement available every fiscal year. Eligible one year from your date of hire. Tuition reimbursement does not carry over if not used.

· Paid time Off (PTO) Entitlement: 240 hours (accrued) Note: PTO accrues upon your first day of employment. You may request vacation after your ninety-day probationary period and have accrued time available.    Time is approved contingent upon supervisor approval.
