Job Description:

Job Summary:
The Clinical Practice Manager is proficient in all tasks performed by clinical staff members. Additionally, the Clinical Practice Manager acts as a role model, trainer, and mentor for clinic staff. Works cooperatively with the Providers, Clinical Practice Supervisor and Nurse Care Coordinator to coordinate and facilitate completion of work required to provide high levels of patient care and service. Performs duties and responsibilities in a manner consistent with our mission, values, and standards.

Essential Functions:

  • Possesses excellent customer service skills and acts as a role model and providing exceptional service.
  • Supervises the Clinical Practice Supervisor, Nurse Care Coordinator, Clinical Support and Referral Coordinator and Lead Referral Medical Assistants
  • Oversees Clinical Practice Supervisor in the management of personnel issues with Medical Assistants, including training, and evaluating performance of other clinical team members. 
  • Coordinates with LPN, RN, Clinical Supervisor and Flow Coordinator to identify and address opportunities to provide better patient care and services.
  • Determines work priorities and assigns work to ensure timely work flow for patient care.
  • Provides directions and monitoring of medication administration by medical assistants.
  • Assists Providers with special tests and procedures within the standard scope of practice.
  • Assist in center with patient care as needed, including, but not limited to, rooming patients, medication refills, referrals, and phone calls.
  • Coordinates with Providers to determine eligibility of patients for Controlled Substance Case Management.
  • Responsible for ensuring patient prescription refills are completed and provides patient medication compliance education and provides appropriate follow-up to specific medication questions is completed.
  • Completes medical prior authorization, as needed.
  • Develops and implements on-boarding orientation, annual training curriculum and competency evaluation protocols for all medical assistants
  • Assumes self -responsibility for all direct patient care activities as indicated by the Clinical Manager within the scope of the RN license.
  • Develops and implement community outreach programs consistent organizational growth strategy.
  • Makes operational decisions as appropriate in the absence of the Chief Medical Director or Clinical Director.

Experience and Skills:

  • Leadership
  • Communication       
  • Counseling skills.
  • Proficiency in communication technologies (email, cell phone, etc.).
  • Strong writing skills and experience developing policies & procedures, clinical training curriculum
  • Highly organized with ability to keep accurate notes and records
  • Experience with health IT systems and reports is desirable.
  • Local knowledge about and connections to community health care and social welfare resources is desirable.


  • Licensed and credentialed Registered Nurse in the state of Pennsylvania.
  • 3-5 years’ experience in clinical or community resource settings
  • Care coordination and/or case management experience is desirable.
